Here are step-by-step instructions for obtaining a medical marijuana registry card in Delaware:
Step 1: Meet with a licensed health care practitioner in Delaware who is an MD, DO, APRN, or PA. Discuss medical marijuana with your health care practitioner and see if they think it would benefit you. Your health care practitioner will complete a health care practitioner certification form if they believe that you would benefit from using medical marijuana.
Step 2: Complete the patient application or the pediatric patient application in full. You can apply online or print out a paper copy of the application. If you need a caregiver, make sure that they also complete a caregiver application. You can find information on how to apply as a caregiver on this website.
Step 3: Pay the application fee, which is $50.00 and non-refundable for everyone who applies. If you are applying online, you can pay the fee through the payment options section. If you are mailing your application, please include a check or money order for $50.00 payable to the State of Delaware.
Step 4: Provide proof of age and residency. Upload a copy of your Delaware-issued driver’s license or state-issued ID with your application. If you are mailing your application, send a clear photocopy of your Delaware-issued driver’s license or state-issued ID to the Division of Public Health at the address below:
